Lumeris Unveils AI-Powered Primary Care Solution 'Tom'
Lumeris has introduced 'Tom', an AI-powered solution aimed at transforming primary care, announced in a press release. This innovative service, unveiled at ViVE 2025 in Nashville, is designed to address the growing gap in primary care availability, which currently leaves 100 million Americans without access to a primary care provider.
Tom functions as an AI-powered care team member, seamlessly integrating into clinical workflows to extend the reach of primary care teams. It autonomously performs tasks such as scheduling, monitoring medication adherence, and conducting post-discharge outreach, thereby reducing the administrative burden on healthcare providers and enhancing patient access to care.
The solution is built on over 20 years of research and five years of AI innovation, utilizing curated data and large language models to deliver personalized care and drive next-best actions. By operating 24/7 within primary care workflows, Tom aims to significantly expand the capacity of care teams, allowing them to serve more patients effectively.
Lumeris has partnered with several leading organizations, including BJC Health System's Center for Health AI and MIT's Computer Science and AI Lab, to further enhance Tom's capabilities and impact. This collaborative approach seeks to create a more accessible, high-quality, and equitable healthcare system across the United States.
