Astrana Health and Physician Association of California Partner to Support Independent Primary Care
Astrana Health and the Physician Association of California announced a collaboration to support independent primary care physicians in California, according to a press release. The partnership aims to help physicians remain independent while improving patient care through Astrana’s AI platform and Medicare Accountable Care Organization programs.
Astrana will provide PAC physicians with access to care coordination, population health tools, and value-based care capabilities. The company’s integrated platform combines care delivery, data, and financial systems to reduce administrative work and enhance patient outcomes.
Participating physicians will gain access to analytics at the point of care, operational support, and financial incentives tied to performance and quality. The collaboration is designed to help small and independent practices better manage Medicare beneficiaries and compete in value-based care arrangements.
Astrana currently operates four ACOs across the MSSP and REACH programs, serving more than 75,000 beneficiaries and supporting over 20,000 providers nationwide.
