PointClickCare has introduced new AI capabilities for skilled nursing facility discharge planning, announced in a press release. The updates aim to give hospitals, health plans, and accountable care organizations a unified view of discharge activities and plan completeness.

The enhancements to PointClickCare's PAC Management solution combine structured discharge assessments with AI-generated insights. Care managers can now access the same discharge assessments used by skilled nursing facilities within their coordination workflows, creating a shared record that reduces duplication and information gaps.

The new AI feature automatically reads and organizes discharge-related information from progress notes and clinical documentation, allowing care teams to view key details while the patient is still in the facility. This reduces manual communication and helps coordinate transitions more efficiently.

PointClickCare will demonstrate the updated PAC Management solution at the AHIP 2026 Annual Conference on June 9 and 10. The company said the tools are designed to help prevent avoidable readmissions by improving visibility and collaboration across care settings.
