Pair Team Joins CMS ACCESS Model to Expand AI Care for Medicare Patients
Pair Team has been accepted as an applicant into the Centers for Medicare & Medicaid Services (CMS) ACCESS Model, announced in a press release. The program is aimed at improving outcomes and lowering costs for Medicare beneficiaries with chronic conditions through coordinated and continuous care supported by technology.
Pair Team’s care model combines clinical services, community-based support, and artificial intelligence to assist patients both inside and outside traditional medical settings. Its AI health advocate, Flora, engages patients by coordinating care, identifying gaps, and addressing medical and social needs in real time.
The company reported measurable results from its approach, including a 52 percent reduction in emergency department visits, a 26 percent drop in inpatient admissions, and improvements in diabetes, blood pressure, and depression management. Pair Team is working with accountable care organizations and healthcare partners as the ACCESS Model prepares for launch.
