Sage Care Launches AI-Powered Care Navigation Platform with $20M Funding
Sage Care has launched publicly with $20 million in funding, led by Yosemite and joined by investors including General Catalyst and Metrodora, announced in a press release.
The company’s AI-powered platform aims to improve healthcare navigation by automating patient triage, specialist matching, and scheduling. Its clinically intelligent agents handle tasks such as 24/7 call triage, appointment management, referral processing, and insurance verification.
Sage Care’s predictive optimization tools integrate into existing hospital systems to reduce bottlenecks and forecast service needs. The system can be customized and deployed within 48 hours to align with an organization’s clinical protocols.
The platform is already in use with partners such as Jiva Health, with upcoming deployments at Bronson Healthcare and White Plains Hospital. Early results show improvements in provider efficiency, reduced no-shows, and fewer unnecessary emergency visits.
