Primary Partners Reports Lower Medicare Costs With Lightbeam Platform
Lightbeam Health Solutions stated in a press release that a one year analysis of patients managed by Primary Partners Alliance found lower costs and healthcare use among those who completed an Annual Wellness Visit. The analysis covered patients in the Medicare Shared Savings Program and ACO REACH.
Patients who completed a visit had healthcare costs of $947 per member per month, compared with $1,014 for those who did not. Their readmission rate was 9%, compared with 16%, while emergency department use was 272 visits per 1,000 patients, compared with 286.
The group that completed visits also recorded a 70% HCC recapture rate, compared with 57% for other patients. Primary Partners uses the Lightbeam platform to identify patient risks and care gaps, prioritize outreach, track performance and support care coordination.
