UT Medical and TransformativeMed Build Agentic AI Discharge Planning Tool

UT Medical and TransformativeMed are partnering to develop Agentic Discharge Planning, an agentic AI tool built on the CORES Platform for Oracle Health EHR workflows.

UT Medical and TransformativeMed announced in a press release a development partnership to build Agentic Discharge Planning for acute care settings.

The solution is part of TransformativeMed's CORES Platform, which the company describes as an Agentic Clinical Operating System. The platform is designed to add coordination and automation inside Oracle Health EHR workflows.

The companies said the project combines UT Medical's clinical operations experience with TransformativeMed's agentic AI capabilities. The tool is focused on discharge planning, a workflow that involves multiple teams across a health system.

UT Medical and TransformativeMed plan to show the solution at the Oracle Health Summit, scheduled for September 22 to 24, 2026.
