Brook.ai and SRHO Partner to Expand AI Remote Care to 275 Hospitals
Brook.ai and SRHO have announced a strategic partnership to deploy Brook's AI remote care platform across more than 275 hospitals in 20 states, according to a press release. The collaboration aims to deliver continuous care infrastructure to SRHO’s member health systems, supporting patients beyond hospital walls.
Brook’s platform combines clinician-supervised AI with remote clinical teams to enable early risk identification and timely interventions. The system is trained on over five million patient interactions and integrates directly into care workflows. Reported results include a 50 percent reduction in hospital readmissions, 74 percent hypertension control, and 71 percent long-term patient retention.
Griffin Health in Connecticut was one of the first SRHO members to implement the platform. The deployment has helped patients better manage chronic conditions such as hypertension and diabetes, with rapid improvements in blood pressure and A1c levels observed within a few months.
The partnership allows community and regional hospitals in the SRHO network to use Brook’s continuous care infrastructure through SRHO’s shared-scale model, expanding access to AI-supported remote care across diverse health systems nationwide.
